Membership application form

Please use the form below to send your details to the Librarian. Individual membership is $250 including GST for twelve months, and payment must be made by credit card. Institutional membership, for example for a whole practice, is not currently available.

When you submit this form your details will be sent to the Librarian. When your membership is authorised, you will be emailed a link where you can pay for your membership. Once you have paid you will be able to access electronic copies of all scientific articles available to the library.

The library also has a bulletin board for discussions of disease outbreaks or unusual cases. Local events will also be posted on this bulletin board.

Enter your full name, as you would like to be referred to in communications (for example John Smith, or Dr. Emily Smith):

Enter the name of your practice, stud, or place of study. If you have retired, please enter "Retired".

Organisation:

Provide a contact phone number in the form 04XXXXXXXX (with no gaps!) and an email address that you check regularly for correspondence.

This email address will also be your login name.

If we cannot get hold of you by phone or email, we may not authorise your membership.
